David Dugan, who was working for Trangie Fire and Rescue on Saturday and attended the scene, said this could be his fourth fatal within 500 metres of the accident in as many years.
Three young men were killed near Trangie in the early hours of Saturday morning after their ute collided with a tree 10kms north of town.
All three men died at the scene, NSW police believe fatigue could have been a factor.
Mr Dugan said the road must have an impact on drivers.
"That has to be the worst bit of road in Australia, they need signs and maybe gutter stoppers to wake the drivers up," he said.
Narromine Shire Mayor, Bill McAnally agrees with Mr Dugan saying the stretch of highway had seen quite a few accidents in recent years and drivers had to be very much aware of fatigue.
During the weekend the three men were identified as Todd Ligar, Mitchell Holloway and Ethan Hertslet from Orange.
The trio was driving outside of Trangie when the 4WD they were travelling in left the road and wrapped around a tree killing the three men, aged 24, 21 and 17.
Emergency services attended the crash scene just before 8am.
Three of the four dogs accompanying the men survived and were treated by a local vet.
NSW Police Highway Patrol Inspector, Phil Brooks, told Fairfax Media fatigue is most likely the cause of the incident as there were no skid marks on the road.
This accident is one of many that occurred over the Anzac Day long weekend.
